HAFAD Seeks Disabled People to Take Part in Survey


Take five minutes to complete the form and you could win £50

HAFAD - Hammersmith and Fulham Action on Disability - are looking for disabled people in West London to take part in a five minute survey.

HAFAD say if you are a user of disability services, including their own services, or are someone who needs a level of care and support but don't know how to go about finding it, you could take part in the survey and be in with a chance of winning £50 worth of vouchers to spend in your choice of store.

HAFAD says the survey the survey is completely confidential and anonymous and you can complete it at HAFAD's website.

Email the completed survey to: darrylbutcher27@gmail.com by the deadline of midnight on June 5 to be in with a chance of winning one of the four prizes of £50 vouchers.

Alternatively, if you prefer to take part in a short phone interview, call Darryl Butcher, HAFAD's Market Research consultant on 07896 611335 and he will arrange a time that is convenient to call you back.
